How do you get a window through 18,000 ft?

I have seen discussions from US sailplane pilots mentioning flights at altitudes of higher than 18,000 ft. I have also heard of hang-gliders and paragliders (in Colorado) going up to altitudes higher than 18,000 feet. Assuming that these pilots are doing so with FAA approval, how are they doing it? Do you radio ATC to get permission? Do you phone them? Are there areas where this "window" is available full-time?
